APPOINTMENTS – BY APPOINTMENT ONLY : )

All appointments are scheduled on-line via square. Once you request an appointment via square you will receive a notification stating your appointment request is under REVIEW; at that time our scheduling coordinator will either APPROVE or DECLINE your request and you will receive a notification updating you on the status. Your appointment has not been confirmed until you receive an approval message. Please verify all information you have provided is correct, all notifications are sent automatically to either your e-mail or through text. We are not responsible for miscommunication due to wrong information provided. If your appointment request is declined we encourage you to try another date/time, if you have any questions concerning our availability please feel free to reach out via text or email. E-mails, Text messages, Calls are for questions but we will not be scheduling appointments through those sources.

APPOINTMENT CONFIRMATIONS

Confirmations are sent 48 hours before your appointment. ALL appointments must be confirmed either through TEXT or EMAIL. Failure to confirm will result in appointment being canceled.

CONSENT FORMS

Please fill out forms 24 hours before the time of your appointment. You can find the forms on the website under the “FORMS” tab – also, please complete COVID19 forms swell. A form must be completed for every service being received. All clients receiving a service must have forms completed.

CONTRAINDICATIONS

Before scheduling an appointment please review our contraindication tab on the website. This tab will advise if you are a candidate to receive services or not. It is solely the clients responsibility to review the risks. Failure to review can result in service not being able to be provided while the appointment is taking place, if so – a fee will be charged to client for service not completed that was scheduled.

CANCELLING / RESCHEDULING / LATE POLICIES

CANCELLATIONS: In the event of a cancellation or reschedule less than 24 HOURS; the credit card on file will be charged 100% of service fee. Reminder emails and text messages are sent 48 hours prior to scheduled appointment time.

NO SHOWS: In the event of NO-SHOW, the credit card on file will be charged for 100% of the service fee.

SICK POLICY: If you are sick, running a fever, feeling unwell or have been around anyone has is sick, PLEASE make sure to cancel or reschedule your appointment in a timely manner.

LATE ARRIVALS : Please arrive ON TIME to your appointment. Arriving late can interfere with achieving the best results. 100% of the service fee will be charged to credit card on file if passed the 5 MINUTE GRACE PERIOD (that is a courtesy) and your appointment will be considered cancelled. My time is reserved specially for you; and to maximize the best results, prompt arrival is needed.

CHILDREN + EXTRA GUEST

Please make sure to come by yourself. No children, pets or extra guest are allowed. If a minor is being serviced, parent or guardian is to be present with minor. No exceptions.

CONTACT INFO

It is your responsibility to verify and update your contact information when changed in order to communicate. At times when appointments need to be confirmed, changed, rescheduled or canceled we will send a text or email to information we have on the system.
